To request a flight with SouthWings, please download the fillable PDF Flight Request Form below, complete the form, save it, and email to either:

Alesha Reardon, for Southern Appalachia
Prior to the flight each passenger will need to sign and return to SouthWings a copy of both the Waiver and Flight Credit Request forms available below. These can be scanned/emailed or faxed to 828.225.7562.

SouthWings is transitioning to a web-based flight mission requesting procedure. Our web-based flight request form is currently in beta testing. If you're interested in filling out the online version of this form instead of the fillable PDF, send us an email and we'll set you up with a username to access it through our website.

- Flight Credit Request Form -- also requires signature
- Passenger Report Form -- to be completed after flight
